I'm making a pre-op spreadsheet of vitamin levels so I can track my post-surgery levels. I've had blood tests for most common vitamins.
What should I include?

Let your labs dictate what you need to up.. I needed D (low end), so I take about 10,000iu D3 a day.. everything else looks good.
I took vites as a normie, and I just take more now.. my normals are:
1 Centrum 1200 Mg Cal citrate (includes some D3 & Mg- Vitalady Tender Cal/mg/D3) 1000mg Vit C B50 complex when I remember, prob every other day B12 sublingual 1000mcg 10,000iu D3 aprox a day Iron- Vitalady's Tender Iron capsule 1x a day Potassium 200mg Omega 3 2000mg Evening Primrose oil 1000mg Biotin
Some things I take just for the heck of it- the biotin I've used for years as it makes my nails nice.. the EPO, helps some female issues.. Omega 3.. just because.. My only low vite was my D level.. and I'm working that one up..

Whoops- saw you were asking for what labs to do, not what pills to swallow.. here's a good labs list compliments of Brandilynn, there may be more but I don't have them in front of me..
Hold vitamin supplements for 3 days prior to lab draw – have them drawn fasting ICD-9 Diagnosis code 783.12 CBC Pro time, PT/INR Iron, TIBC, ferritin, transferrin Copper, Zinc, albumin Comprehensive metabolic panel Folate, Vitamin A (DO NOT EXPOSE TO LIGHT), *vitamin B 1, *vitamin B6 (*also do not expose to light), vitamin B12, vitamin D (Dhydroxy 25, 0,25 – NOT 1,25) PTH (for use with calcium and vitamin D levels, where is calcium coming from) Lipid panel, triglycerides, LDL, VLDL, HDL, cholesterol total.
This is what I was given, minus the PTH and the light information on the B1 and B6, this I learned after my first set of labs and asked for help.

Just to clarify the good list jo already posted, the last line includes the elements already included in a lipid panel - be careful not to request, for example, a lipid panel AND triglycerides because then insurance may not cover both due to duplication (I ran into this issue recently with something else my doc unintentionally duplicated on a lab order).
I also request a vit E test (tocopherols), and, since I've been type 2 diabetic, an HBA1C and a fasting insulin, which you may also want to include if you've veer been diabetic or insulin resistant.
