Dated photographs are helpful, but where they need to be documented is at your pcp.
Have it in his records along with all the unsuccessful treatments.
Not sure what length of time your insurance might require to prove to them that the only solution is surgery. But, I'm sure the longer the better.

I had my plastics years ago but I never documented anything. I had consults with two plastic surgeons. One was very reluctant to submit to insurance but on my insistence she did. It was denied immediately. I then went to another plastic surgeon who was very familiar with bariatric surgery (several of my fellow DS and VSG friends went to him). He took lots of pictures and wrote a great medical necessity letter. It was approved.
With that said, please have your PCP document everything - pictures, notes, etc. The more ammo you have the more likely insurance may pay for your TT.
