I'm a dummy, but I can't find his Vitamin A level. Am I missing it? And without seeing his parathyroid (PTH), his calcium and D levels are irrelevant to me. Calcium, D and PTH are all in it together, like a three legged stool. Take one out, and it's not complete.
I have to "tweak" my vitamins after every lab test so far, and I'm about 2.5 years out. If I was compounding my vites, I'd have gone insane by now. Buying my own and keeping track myself if easy. Communicating to a pharmacy and the chance of error? Not worth it to me.
Also, when you are taking mega-doses trying to raise a low level, you should (IMHO) have a lab done on that level every 3 months. I had an experience with my D and will tell you about it in a PM if you want.
